Susan’s Heart is a home health care agency offering compassionate and professional senior care services in Baltimore, Maryland. Some of our offerings include medication reminders and management, meal preparation, chore services, errands, light housekeeping, hygiene help, companion care, and many more. Our skilled offerings include skilling nursing, physical therapy, occupational therapy, and speech therapy. All of our services are performed from the comfort of your own home whenever you need them; 24 hours a day and 7 days a week.
In order to provide the right level of care to our patients, we coordinate with our patients’ physicians and/or local health department while working in tandem with the supervision of a highly skilled registered nurse. Your healthcare care assessment and plan will be individualized to you specifically and will be implemented and updated throughout the care process to ensure that we are able to meet your needs and preferences.
